Patient Confidentiality Agreement
This Agreement records the confidentiality commitment between Kawenge Health Services (KHS) and you as a patient. By accepting it during registration, or when signing your bio-data form, you and KHS agree to the following.
1. Confidentiality obligation of KHS
All medical information you share with KHS — symptoms, diagnoses, prescriptions, vitals, test results, images, notes — will be treated as strictly confidential. Access is limited to clinicians directly involved in your care and to a small number of authorised administrative staff bound by the same confidentiality duties.
2. Patient autonomy
You have full autonomy over how your medical data is used by KHS. You may:
- ask us who has accessed your record;
- request a copy of your record;
- correct information you believe is inaccurate;
- withdraw permission for any specific use of your data at any time.
3. Sharing with third parties
KHS will not share your medical record with any third party (employer, insurer, family, another clinic) without your explicit written or in-app consent, except where compelled by a valid PNG court order or law.
4. Storage of data
Your medical data is stored encrypted in KHS's managed backend, with role-based access controls and audit logging, in line with our Privacy Policy.
5. Medical code of ethics
KHS clinicians follow the medical code of ethics applicable to registered medical practitioners in Papua New Guinea, including the duty of confidentiality and the duty of care.
6. Your commitment
You agree to provide truthful bio-data and medical information so that we can deliver safe care. You agree not to record or share consultation footage without the doctor's consent.
7. Duration and withdrawal
This Agreement applies for as long as you hold a KHS account. You may withdraw at any time by emailing info@kawengehealth.com. Withdrawal does not affect processing already carried out with your prior consent, and does not extinguish record-keeping obligations required by law.
